BJC Behavioral Health has been selected to be a Healthcare Home for St. Louis City, St. Louis County, and the counties of St. Francois, Iron, and Washington, Missouri.
A Healthcare Home is a “whole person” approach of delivering health care, which includes managing a full array of physical and behavioral health needs as well as long-term community care services and supports. Healthcare Homes focus on wellness and prevention as well as treatment and rehabilitation. This team approach has been shown to improve chronic health conditions and wellness.

Who is eligible to be served by a CMHC Healthcare Home?
Individuals covered by MO HealthNet are eligible to be served by a CMHC Healthcare Home if they have:
- A serious mental illness (including children and adults receiving psychiatric rehabilitation services under the Medicaid Rehabilitation Option), or
- A mental health condition and a substance abuse disorder, or
- A mental health condition or a substance abuse disorder, and one of the following chronic conditions or risk factors:
- Cardiovascular disease
- Developmental disability
- Overweight (BMI >25)
- Use tobacco
