Timely communication is the key to successful treatment. A client's case manager is at the hub of that communication and makes sure that all parties -- including the client's support network -- are notified of progress and setbacks. Failure to communicate can result in non-payment. It is important that the client's case manager or supervisor be notified immediately by phone or fax for:
- Missed appointments
- Need for hospitalization; hospital admission and hospital discharge
- If the case manager is not directly involved with admitting the client to the hospital, please notify immediately as to:
- Where the client is hospitalized
- Estimated length of stay
- With this information, the case manager can help with discharge planning
- Any change in current condition that requires a change in the frequency of visits
- Any unusual incident or significant treatment changes:
- Death or serious injury
- Reasonable cause to believe there has been neglect, or physical or sexual abuse
- Any unusual incident requiring a significant response or intervention
- Use of restraints
- Behavioral crises including:
- Problematic or disruptive behaviors
- Under the influence of alcohol or drugs
- Harassment and threats
- Suicide attempts
- Assault
- Arrest
- Medical emergencies
- Newly diagnosed serious illnesses
- Involuntary commitment
- Any situation that may come to the attention of the news media
